           People who have similar attitudes and mind-sets will serve to inspire you and help you achieve the goals you have set.

           Well, it’s a busy time of life! People over forty have a very busy work life, they are often at the peak of their career, have a young to teenage family, and quite often don’t feel they can exercise because they don’t have time for it. They have less energy, feel tired, and often are not actually sleeping enough, which can be a major contributing factor and one frequently overlooked. The over forty set have issues with regards to reduced general flexibility, stiffness, poor posture - and they have aches and pains they have never had before!

  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;

           By the time you hit forty ... you are starting to feel the effects of gravity!

Your spinal posture is closely linked with everything - ie. what you are doing with your spine is linked to your problems with your shoulders, which of course affects what happens with the rest of your arms.            Is Physical Work Really Enough Exercise?

So, regardless of what someone does for a living, they can often develop very similar problems for very many reasons - as time goes by. Frequently we see at the centre, tradespersons or people with very physical jobs who believe they do not need to go to the gym or do any exercise at all because they do enough at work. But in effect, they are spending all day doing ‘bad exercise’ and really need to counteract that with ‘good exercise’!

This is compounded by the fact that they go to the doctor who may prescribe anti inflammatory, cholesterol or blood pressure pills, where appropriate, which may offer some relief but does not address the actual source of the problem. So a lot of people believe that this form of treatment is going to ‘sort out the problem’, and sadly don’t attempt to actually solve the cause of the problem - which of course will make a big difference in the long term.

  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           Unfortunately we see many clients who have had an innumerable amount of tests and scans before even being assessed by a physiotherapist - who could possibly provide a more accurate determination of the source of the problem. At the centre, we are able to refer our clients for X-rays and scans when we feel it is appropriate, and will refer them to the doctor or an appointment specialist when necessary. We always recommend everybody get a full health check, but where there is a continual effect of reduced flexibility, stiffness and poor posture, a visit to the physio could address the source of the problem - and make life easier - over time!

           Probably people want to know whether it is ‘normal’ for them to have symptoms such as intermittent headaches, stiffness in their back, or difficulty doing certain activities such as bending. They naturally then want to know what, if anything, can be done about it!
          &#xD;

  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      &lt;br/&gt;&#xD;
    &lt;/span&gt;&#xD;

    &lt;span&gt;&#xD;
      
           Seeking Advice and Reassurance
          &#xD;

           Quite often it has been a gradual onset of problems over a long period of time. Seeking advice or help they get told by their doctor or friends, or their family “You are just old and don’t worry, you’ll be right mate.”. The Internet is another source of information where people look for remedies and quite often incorrectly self-diagnose their problem. So because of all the conflicting opinions they have been given from everyone and everything around them, they really don’t quite know what they are able to do to sort out the problem. 
           &#xD;
      &lt;br/&gt;&#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      &lt;br/&gt;&#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           What’s The Real Problem?
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      &lt;br/&gt;&#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           As physios we discover, what they really want is reassurance and to know that there is something there that can be done to help them - and they want to know ultimately how to go about it. Overwhelmingly, we note that most everybody is concerned that their general fitness, feeling that it is not what it should be. As people age, they can feel that they have slowed down quite a bit and they realise that they are not putting in the effort of the past. This, of course, is quite common and can depend to a large effect on what they have done throughout their lives. Apart from a someone who has been less active, if they have been a sportsperson, or had a very physical job, or have injured themselves in the past - these things can also contribute largely to the problem at hand, and become more of an issue as life goes on.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      &lt;br/&gt;&#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           The Domino Effect 
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      &lt;br/&gt;&#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;

    &lt;span&gt;&#xD;
      
           The domino effect of this is that it tends to lead to less mobility, decreased fitness and less energy. Fears of osteoporosis, osteoarthritis and diabetes, especially if there is a family history, can become a real concern. Because there is so much conflicting information out there, they don’t really know who to trust to help them. Everyone has a different opinion and they don’t really know who or what advice to follow. Adding to this, are also time and costs constraints, a real concern that they are not going to have enough time or money to do the things necessary to fix the problem.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      &lt;br/&gt;&#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           The Good News
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      &lt;br/&gt;&#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           But the good news is that physios are aware of these concerns and can help formulate a plan that is going to fit in with your lifestyle and budget. Physios can help with long term or chronic pain and help with knowledge to understand the underlying problems so that they can dealt with in a person’s everyday life. It can be a case of getting out of old habits, changing mindset, setting goals and getting on with it by avoiding delay by addressing the problem.

  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      &lt;br/&gt;&#xD;

           One thing that physios treat that people aren’t always aware of - is feet! The truth of the matter is that we like feet! They are, after all, a very crucial part of our anatomy. If someone has significant biomechanical issues with their feet this can cause problems in all areas from the feet, to the knees, to the hips and spine. We treat heel spurs, bunions, plantar fasciitis and Achilles tendonitis as well as a number of less common foot conditions. 
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      &lt;br/&gt;&#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           We have a computerised Gait Scan Analysis machine with which we can comprehensively assess foot posture as well as prescribing and supplying custom made orthotics where necessary. We can also give excellent advice about the best type of shoes suitable for the condition presenting, and from where they can be sourced. The application of improved and better suited footware can lead to extra comfort and support for the two things that carry you through daily life!
          &#xD;

  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;

           Shocking But True!

           Other less obvious areas for treatment are the sub-acute, sub-chronic and chronic conditions of the myofascial trigger points and the tendinopathies including plantar fasciitis, tennis elbow and Achilles tendinopathy. A less known mode of treatment used by physiotherapists is the use of Shockwave Therapy. This form of treatment can effectively assist in the promotion of healing in these areas.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      &lt;br/&gt;&#xD;
    &lt;/span&gt;&#xD;

           Using Shockwave Therapy, biological effects can be triggered through radial shockwaves, which leads to faster and permanent healing and regeneration of the body’s soft tissues, tendons, muscles and bones. The body’s painful parts can be targetted through the machine’s transmitter at the end of the applicator via compressed air which carries high energy through radial shockwaves to the affected areas. The energy transmitted can promote repair and regeneration, and promote activation of muscle and connective tissue by improving circulation and muscle tone. 
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      &lt;br/&gt;&#xD;

It is recommended that patients trial at least 6-8 sessions of shockwave therapy with approximately 4 days in between sessions to see a good result.

           Should You See a Physiotherapist After a Serious Accident?

           As physiotherapists, we get to treat many clients after serious road or work accidents. Physiotherapy treatment provides relief for injured clients, but encouragement to commence a rehabilitation gym program is also very important. Exercise programs as part of a rehabilitation regime can be of great benefit, but they should be undertaken with specialised attention.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      &lt;br/&gt;&#xD;
    &lt;/span&gt;&#xD;

           Dorothea

            had a car accident and severely injured her left knee and her neck. As part of her rehabilitation she came to the gym with me some months after the accident, having never been to the gym before. She is now exercising regularly and had lost a lot of weight and is looking and feeling great. She is still having ongoing maintenance treatment once a month to keep her chronic problems at a manageable level and is doing very well.

           , who is a concreter, came to me after he fell off his off-road bike and fractured his neck. Initially, as he had just come out of his neck brace we had to very slowly progress his rehabilitation. After three months I then commenced a gym program with him which we gradually progressed and he has become a regular gym member. Ironically he always had chronic lower back pain, long before he broke his neck, and now as a result of going to the gym he is fitter than he has ever been. His back gives him less pain than it has ever done before and above all he has more energy. He has two young children, which he can go home to in the evenings and have fun with. He is more optimistic that he can remain in his career for longer as it is one that is a very difficult career for somebody that has these injuries.
          &#xD;

  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      &lt;br/&gt;&#xD;

           Darren
          &#xD;

           , who is in his forties, comes to mind. He is a painter who works in commercial and domestic painting. He came to me working six days a week and his body was in trouble! He had chronic back pain, chronic shoulder pain and Achilles tendonitis. He was doing no exercise at all besides the strenuous activities of his work. I have been seeing him now for over a year, but within a matter of weeks we were relieving a great deal of his symptoms. With our hands on treatment and stretching regime we released a lot of his pain. We then encouraged Darren to join the gym. He attends the gym regularly and has lost a significant amount of weight. His fitness has increased significantly and overall his symptoms have been significantly reduced. He is able to work a full day without the difficulties he was previously having. He has been of great inspiration to me!

Due to all of the travel associated with his job and erratic meal times, he has put on weight over time. He was very busy, very stressed and wasn’t doing any exercise at all. He had operations on both his knees, operations on his shoulder and he has chronic neck and back pain. As well as having regular maintenance physiotherapy with us, he has now joined the gym and he has become a regular exerciser. He has lost more than fifteen kilos and by doing all of that he has significantly relieved his symptoms. He is bike riding with the kids and is able to bike ride to and from work as well. He is not taking quite as much pain medication which is important because he has issues with his stomach too. He is such an inspiration because even with his incredibly busy lifestyle, he has the right mental focus and has achieved a great deal for himself.

She also walks with a good friend of hers, they motivate each other and she has a home program to do as well.            Do We Need To Do Sit Ups?

           Unfortunately, there is a significant amount of evidence out there which proves that both abdominal machines and crunches can increase your risk of injury, not only primarily to your lower back but also to the rest of your spine. This, of course, becomes an even greater risk once we are over 40 and have had a few underlying problems. There have been many trends over the decades from sit ups to crunches to ball crunches. The word that you hear around at present is ‘core’ strengthening - the entire midsection. The latest research on the ‘core’ shows what boxers, martial arts and dancers have said for generations - that strength comes from the centre. 
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      &lt;br/&gt;&#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           With our strengthening, we focus on training muscles, not in isolation but to duplicate functional movements from normal daily activities. This means exercises that challenge the whole body without putting excessive strain on joints, discs, muscles or ligaments. The good news is that this means you do not have to do sit ups, and in fact by doing a workout full of dynamic movements targeting the legs, arms and back you get the best workout of all.
          &#xD;

           Should We Do Sit Ups Over 40?

           Once a female is over 40, has had a few children and has a few pelvic floor problems they SHOULD NEVER DO SIT UPS OR CRUNCHES, as these cause significant problems due to the extreme intra-abdominal pressure that is generated. The rows of machines that you see at the gym are not all necessary and in many instances can cause significant injury - even when used ‘correctly’. There are many other exercises that achieve a better result without any of these problems, by selecting a regime that is designed to specifically suit you. 
          &#xD;

  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      &lt;br/&gt;&#xD;

           Exercise Programs Can Benefit Women Over 40
          &#xD;

           Many women over 40 need very individual care with regards to exercise and self-management.

      &lt;br/&gt;&#xD;
    &lt;/span&gt;&#xD;

           Following are a couple of interesting case studies:

           Klara is in her mid-forties and she has been living with rheumatoid arthritis for over ten years. Even though she knows it is going to be a constant battle, what she has currently achieved is a way of at least being proactive with her disease process. She goes to the pool on a weekly basis and has done so for about eight years now. With that type of exercise regime, she has been able to maintain a good level of function overall and even though she does have ups and downs she is able to manage living on her own at home. She has recently retired from her job due to her arthritis but she is able to take care of her animals and do most things she has to do. The other advantage of the pool for her is that it gives her good support and encouragement when she is having a bad week, and as well there is a very big social aspect to undertaking group exercise. Group exercise is another good way to get out of the house and do something active and positive with other people. Klara does have ongoing treatment depending on how she is going, but on average about once a month she will come into the centre and we will address whatever issues may be concerning her.
          &#xD;

           These days physiotherapy has a wide variety of modalities used to treat musculoskeletal problems and other conditions seen as assessment. The choice of modality is decided on an individual basis. Physiotherapists that utilise a hands-on approach to treatment apply manipulation and mobilisation techniques to loosen stiff joints, as well as soft tissue and massage work. These techniques can be very gentle if appropriate and with experience can manage to treat even very osteoporotic or severe arthritic conditions without any of the dangers associated with vigorous manipulation or massage techniques.

           It is very common for people to be told that the pain they are experiencing is muscular in origin, especially with spinal complaints. With the vast majority of spinal pain, the problems can arise from an underlying pathology. This may be due to stiff joints, osteoarthritis or intervertebral disc pathology, which in turn can lead to significant changes in muscle spasm, function and weakness, ligament inflammation and neural irritation. As a consequence, it is quite vital to diagnose the problem correctly and treat the pathology, as well as the subsequent arising problems. Starting with some basic stretches and strengthening exercises, the program can carefully progress as the client is able to advance.

This will ensure that they do not re-injure themselves by going to the gym or a personal trainer too soon, or by just going back to sport or exercise too early.
